Standard of play questionnaire

This questionnaire is designed to give us an idea of your standard of play. We will use it to help us match you with players of a similar standard when you join the league. It's also where we ask you for your contact information and offer you the opportunity to sign up for FREE British Tennis Membership!

The information submitted will not be shared with third parties with the exception of your email and phone number which will be shared with other tennis players in any group you are placed in, and with the LTA for the purpose of British Tennis Membership (subject to you granting permision for us to do so, below). By submitting this form you are granting us permission to use this information in this way.

The best address for posting free, bonus tennis balls. This might need not be your home address, it could, be a work address

The address is for prizes/balls: It can be a work address if no one is in during the day. Your address will not be passed on to others in the league.

This is the email address that will be shared with other players in your group for the purpose of setting up matches and used by us to send you information about the league. It will not be passed to third parties.

If you haven't already paid, return to the Join Now page of the league you want to join (use the league menu on the right) after you have completed the questionnaire. You will have the option of paying securely by PayPal or in formation for paying by cheque. If you would like to pay another way, including bank transfer, contact localtennisleagues@googlemail.com. You will not be allocated a place in the round until we have received a payment!

By giving your consent to you becoming a British Tennis Member, you agree that you will abide by the Terms and Conditions of British Tennis Membership (at www.LTA.org.uk/BTMTC) and that the LTA and its directly affiliated bodies (see www.LTA.org.uk/affiliatedbodies) can use the personal data of you, including sensitive personal data that you provide, for the purposes of your involvement in British Tennis, and to send you/your child by post, e-mail or SMS information related to those purposes (for more information see www.LTA.org.uk/privacy).
All players in a Local Tennis League are eligible for FREE British Tennis Membership. Please indicate whether you would like us to submit your details to the LTA.
If you think you may already have a British Tennis Membership, or a lapsed LTA rating let us know here. Please give us as much information as possible. This will help the LTA avoid duplicate entries.
